Common causes include:

Wrong bit size: a bit that is too small or too large cannot fully engage the screw recess

Incorrect bit profile: Phillips, JIS, Torx, Pentalobe, Tri-point, and other profiles are not interchangeable

Poor tip machining: inaccurate geometry creates limited contact and increases slipping

Excessive torque: too much torque can deform or round the screw head

Insufficient downward pressure: the bit can climb out of the screw recess during tightening

Driving at an angle: side loading reduces contact area and increases cam-out

Worn screwdriver bit: rounded or damaged edges cannot transfer torque properly

Low-quality screw: soft screw material can deform even when the correct bit is used

High speed on small screws: excessive RPM can reduce control and increase slipping

Repeated installation and removal: already worn screw heads are easier to strip
